29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about January 25, 2018 the employer did not ensure that employees who perform maintenance on the Haskel Air Driven Liquid Pumps (model No. ALC-15) were adequately trained in the procedures for the safe application of lock out tag out. Employees were exposed to amputation hazards from the unexpected star-up of the equipment while removing and replacing the Haskel Pumps. b) Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about February 2, 2018 the employer did not ensure that employees who performed troubleshooting tasks on the Haskel Air Driven Liquid Pumps (model No. ALC-15, serial No. 57061109) were adequately trained in the procedures for the safe application of lock out tag out. Employees were exposed to amputation injuries from the unexpected start-up of the equipment.
