29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about January 25, 2018 the employer did not ensure that energy control application steps to control pneumatic energy sources were implemented prior to performing maintenance on the Haskel Air Driven Liquid Pumps (model No. ALC-15). Employees were exposed to amputation injuries in that pneumatic energy sources were not isolated. b) Nanogate Jay Systems, LLC. located at 150 East Longview Avenue in Mansfield, Ohio; On or about February 2, 2018 the employer did not ensure that energy control application steps to control pneumatic energy sources were implemented prior to troubleshooting tasks on the Haskel Air Driven Liquid Pumps (model No. ALC-15, serial No. 57061109). Employees were exposed to amputation injuries in that pneumatic energy sources were not isolated.
