Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about January 9, 2018, an employee was exposed to amputation hazards while attempting to unclog an ink line due to the work was conducted by an IEP (Intermediate Energy State) process, and did not follow the established procedures and sequence for the application of energy control (the lockout or tag out procedures) to isolate the rotation of an ink cylinder on the 5054 Ward Flexo Printing Machine in a box manufacturing assembly line: -The IEP process did not apply a lockout or tag out device to an energy isolating device. -The IEP process did not verify that isolation and deenergization was accomplished.
