Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a. On or about January 2, 2018, two authorized employees were involved in the maintenance activity of repairing a caustic line valve which restricted the flow of fluid into the Whole Flow stage 2 tank. The employer did not ensure that the authorized employees implemented energy control application steps as the equipment was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving hazardous energy isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect the employees from caught-in and/or acid splash hazards. b. On or about January 2, 2018, and at times thereto, authorized maintenance employees would perform service and maintenance on fluid valves throughout the manufacturing facility. The employer did not ensure that employees applied a lock to the isolation points [per the 1910.147(d)(4) requirements]. As a result, the authorized employee did not maintain exclusive control of the hazardous energy sources in order to protect them from caught-in and/or acid splash hazards.
