Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls, crushing and struck-by hazards from unbraced concrete forms: Location: Jobsite, Building C, Column Z26.5 On or about November 20, 2019, an employee sustained injuries when and inadequately supported concrete column form fell over, pinning the employee beneath it. Among other methods, one feasible and acceptable means of abatement would be for foreman to do regular and frequent inspections of the column forms to confirm that they are adequately braced to support all reasonably anticipated loads to be applied to the form.
