On or about 11/16/2017, at the Double R Brand Foods facility located in Lufkin Texas, the employer did not establish and implement written procedures to manage changes to equipment, including but not limited to. a) Control panel for the compressor #10, b) Two new emergency stop buttons located outside the engine room door entrances (West and South), C) Two new visible alarms located outside the engine room door entrances (West and South).
