29 CFR 1910.119(j)(4)(i): The employer did not perform inspections and tests on process equipment. On or about November 16, 2017, at the Double R Brand Foods facility located in Lufkin Texas, the employer did not perform inspections and testing on process equipment for the ammonia refrigeration systems, including process equipment such as, but not limited to: a) Ammonia detectors, b) Visual and audible alarms, c) Exhaust ventilation fans, d) Oil Pressure Differential Cut Out for the compressor #2, e. High and Low Pressure Cut Out for the compressor #10, f. High temperature Cut out for the compressor #10, g. High and Low Level Controls for Re-circulator V-1, h. Emergency Stop Buttons for the ammonia refrigeration system, i. Liquid King Valve for the high pressure receiver, j. Re-circulator V-1, k. Re-circulator/ Intercooler V-4, l. Control Pressure Receiver V-6, m. Evaporative condenser EC-4, n. Compressor # 2, o. Compressor # 10, p. Re-circulator Pump AP-1 for V-1, q. Re-circulator/Intercooler Pump AP-8 for V-4, r. Visual inspection and non-destructive testing for ammonia refrigeration piping system.
