29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Pallet Mill; On or about November 14, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on the Baker 5 Head Saw to replace blades without shutting it down and locking it out in accordance with the company's Lockout program established procedures.
