29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Fresh Products, LLC - Perrysburg, Ohio: On or about November 3, 2017, employees were exposed to pinch point and amputation injury in that the Doman Industries (#3) wrapper machine (s/n 78-19950) was not de-energized prior servicing tasks such as, but not limited to, setting up/threading, and unjamming the machine. The employer did not ensure that energy control application steps were implemented to control hazardous energy prior to the servicing task; the employer did not: - Ensure employees had the method or means to control hazardous energy. - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control the energy source(s). - Verify that isolation and de-energization of the machine or equipment had been accomplished. b. Fresh Products, LLC - Perrysburg, Ohio: On or about November 6, 2017, employees were exposed to rotating parts and amputation injury in that the Doman Industries (#3) wrapper machine (s/n 78-19950) was not de-energized prior to servicing tasks such as, but not limited to, re-aligning the machine's jaws. The employer did not ensure that energy control application steps were implemented to control hazardous energy prior to the servicing task; the employer did not: - Ensure employees had the method or means to control hazardous energy. - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control the energy source(s). - Verify that isolation and de-energization of the machine or equipment had been accomplished.
