Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about November 1, 2017, an employee opened the lid to the hopper to perform an inspection related to a malfunction of material flow and was not required to perform the following actions to isolate hazardous energy sources (lockout): a) turn off and shut down the machine, b) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), c) affix lockout or tagout devices to each energy isolating device, d) verify that isolation and de-energization of the machines or equipment have been accomplished. Employees were exposed to amputation and laceration hazards when opening the hopper door for cleaning or checking the levels of material. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
