29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) 400 North Weaber Street, Annville Pennsylvania - An employee was hospitalized due to a fracture of the hip/femur sustained from a fall off a ladder, on October 24, 2017. The employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this amputation to OSHA on October 27, 2017.) No abatement certification or documentation required.
