29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Zara Construction, Inc. on or about September 22, 2017, at the address of a customers home in Butler Ohio: The employer failed to report an employee amputation following a worksite incident (laceration later resulting in an amputation). The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employee's hospitalization within 24 hours of admittance. The employer later contacted the OSHA Office on October 23, 2017.
