Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to severe bodily injuries and/or death from falling objects and/or being caught between the load bearing means and the wheel of a John Deere 304J loader machine: ESTABLISHMENT: The employer did not ensure through providing specific trainings and/or instructions for each job task and/or visual observation that the employees engaged in the "tasks", such as (but not limited to) maintenance, servicing, repairing and/or others on John Deere 304J Loader machine were performing the "tasks" safely at all times. On or about 10/26/17, the hydraulic line burst causing the fork attachment slid to the ground and the load bearing means fell on to an employee who was manually checking a leak from the hydraulic line of the above machine while standing between the load bearing means and inside of the driver's side front wheel. The employee sustained a fractured left collar bone, cracked ribs and a collapsed lung from the incident.
