29 CFR 1918.24(b): When any fixed ladder is visibly unsafe (or known to be unsafe), the employer did not identify such ladder and prohibit its use by employees: a) Mooring dock ladder located at the Barry Steam Plant's marine terminal: On or about October 7, 2017 and at times prior there to, the employer did not ensure the back clearance was adequate the entire length which exposed employees to injuries associated with striking inanimate objects while falling approximately nine (9) feet six (6) inches to the water below. b) Mooring dock ladder located at the Barry Steam Plant's marine terminal: On or about October 7, 2017 and at times prior there to, the employer did not ensure the face of the ladder was not slanting back which exposed employees to injuries associated with striking inanimate objects while falling approximately nine (9) feet six (6) inches to the water below.
