Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that precautions were not taken to eliminate an explosion hazard during the transfer of liquified petroleum gas (i.e. propane) in accordance with the National Fire Codes (NFPA 58, Chapter 4, Sections 4-2.1.1 and 4-2.3.2(c)), and with 49 CFR 171.5(a)(1)(v). (a) 9985 North Pinal, Spaces 131 and 132, Casa Grande, AZ 85222: Employees were exposed to an explosion hazard while transferring liquified petroleum gas (i.e. propane) in that 1) all persons employed in handling LP-Gases were not trained in the proper handling and operation procedures and the employer did not document that training, 2) sources of ignition were not removed, and 3) there was no written emergency operating procedure located in or on the truck, nor were employees trained in its provisions.
