Text For Citation: 01 Item/Group: 001A Hazard:
Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: (a) On or about October 3, 2017 and at time prior to that date, for the employees exposed to amputation hazards while operating the Leland-Gifford, 6 - spindle drill press, model no. 2LMS, serial no. 11063 where the run control foot pedal was not covered to prevent inadvertent operation in the fabrication department at 237 NW 12th Street, West Fargo, North Dakota. Among other methods, one feasible and acceptable method to correct this hazard is to provide a guard or cover for the run control foot pedal that meets the requirement of the American National Standards Institute (ANSI) B11.8 - 2001 (R2012), Safety Requirements for Manual Milling, Drilling and Boring Machines with or without Automatic Control, Section 6 Design and construction, and specifically 6.14.6 Run control devices, which states: Run control devices shall be used to start continuous machine operation and shall be guarded to prevent the accidental starting of the machine.
