29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources: a) Building 47, 30 Curry Avenue, Canonsburg, PA 15317: On or about September 28, 2017, a potential transformer that was contained within a substation style transformer (serial number R-08718-1) was not disconnected from all electric energy sources. A work bench used to provide power a load tap changer (LTC) motor also provided power to the potential transformer. The potential transformer converted the power to approximately 2000 volts on the high voltage side. Employees performing ratio and resistance testing were exposed to the hazards of electric shock and electrocution while working in close proximity to the potential transformer bushings.
