Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(a)(2)(ii)(B): Lockout/tag out procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation: (a) The Cut-Up Area of the facility - On September 5, 2017, a Maintenance Employee was exposed to serious injuries to the hands and or fingers, while engaged in bagger maintenance activities on the Ken-Mac Bagger Machine, Model 93500. The employee placed his hand into the Point of Operation of the live machine where none of energy sources had been locked and tagged out. As a result, the employee received an amputation injury to his left middle finger.
