29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report: a. On or before September 11, 2017, the employer did not use the OSHA 300 Log of Work-Related Injuries and Illnesses or an equivalent form for recordable injuries and illnesses in the workplace. The employer's 2017 OSHA/Worker's Compensation Claims log did not contain the employer's city and state, where the event occurred, columns for classifying the case equivalent to OSHA's columns G through J, columns for the injury or type of illness equivalent to OSHA's columns M(1) through M(6), separate logs for each year, and the page totals.
