Text For Citation: 02 Item/Group: 001B Hazard:
29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Lakeview Farms, LLC located at 1700 Gressel Drive in Delphos, Ohio; On or about August 28. 2017, the employer did not ensure that employees utilized energy control procedures on the 4-Out ATS Filling Machine located in the Gel Processing room prior to conducting set-up procedures. Filling manifolds were not isolated from the electrical and pneumatic energy sources prior to engaging in these activities. Employees were exposed to pinch point and amputation injuries in that electrical and pneumatic energy sources were not isolated. Lakeview Farms, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(i) which was contained in OSHA inspection number 1082557, citation number 1, item number 3 and was affirmed as a final order on October 15, 2015 with respect to a workplace located at 1700 Gressel Drive in Delphos, Ohio 45833.
