29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): Lakeview Farms, LLC located at 1700 Gressel Drive in Delphos, Ohio; On or about August 28, 2017, the employer did not ensure that employees group lockout devices were affixed to energy sources during set-up activities on the 4-Out ATS Filling Machine in the Gel Processing Room. Employees engaged in set-up activities were exposed to hazards from rotating manifold valves and a moving conveyor.
