29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished. a) Department 1103, 1 Second Street, Kreamer, PA - Employees cleared jams from the Weinig Powermat 1000 (Company Name A1-514) without utilizing the machine's lock-out/tag-out procedure or verifying that the cutting heads had been de-energized exposing employees to caught-in and struck by-hazards that could result in an amputation, on or about August 18, 2017. In accordance with 1903.19, abatement certification is required for this violation.
