Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Parallel Products of Colorado, LLC at 123 9th Ave., Greeley, CO 80631: On and before August 18, 2017, the employer did not ensure that specific procedural steps were developed, documented, and utilized for shutting down and isolating hazardous energy for equipment such as, but not limited to, the baler, auger and can crusher. An employee participated in greasing the upper part of the auger by climbing up on the adjacent baler, and standing on top of it. The baler activated, and caught the employee's foot on the baler ejector chain, resulting in a partial amputation of the employee's foot. This condition resulted in exposure to an amputation hazard.
