29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a.) On August 1, 2017, the employer did not ensure that all residual energy was restrained while performing maintenance on the side arm of the #2401 McNeilus automated side loader refuse truck (Model 42). An employee was performing maintenance on the raised hydraulic side arm, when it unexpectedly released pressure and came down on the employees arm, resulting in crushing injuries.
