Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Jobsite, PNC Firstside Café, 500 1st Avenue, Pittsburgh, PA: On or about August 4, 2017 employees had not received training in the recognition of applicable hazardous energy sources or the methods and means necessary for energy isolation. An employee who was changing an attachment on the Hobart mixer, Model D330, did not know the method or means to control the energy prior to turning off the machine to make the change. The employee did not know that the equipment was cord and plug and had not removed the equipment from the power source prior to making the change. The employee was installing a grating attachment to the machine when it suddenly turned on and pulled the employees hand into the rotating blade. The employee sustained amputation to the middle finger of their left hand.
