29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) On or about August 9, 2017, and in times prior thereto, the employer did not ensure the jobsite and equipment were inspected frequently and regularly by persons competent to perform such inspections to ensure defective equipment and/or practices were detected and removed. For example, equipment involving the collapse of a wall panel: the outside section on the telescoping pipes used to brace concrete panels was field made. No factory holes for the adjustment pins were provided. An all thread bolt was used instead of the manufacturer's pin and the one much too large torch cut hole used did not have an inside pipe to connect to. The rigger was without a visual reference to see if the inside section of the telescoping pipe support was connected to the outside section resulting in the two pieces not being connected, exposing employees to the hazards of being struck by an unsupported concrete panel. In addition, multiple field modified braces with replacement bolts not approved by the manufacturer were used.
