29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; Such activities were not performed only by experienced and trained employees selected for such work by the competent person: a) On or about July 27, 2017 and at times prior, Quest Masonry, Inc., as the creating and correcting employer, did not ensure that the scaffold was erected or altered by trained and experienced employees. Outrigger planks were not nailed, bolted, or secured to the outriggers; each abutted end of the outer planks was not rested on a supported surface; and guardrail systems were not installed along all open sides of the platform. A foreman of Quest Masonry Inc. and Funez Stucco LLC were working on each end of the outer planks when one of the workers moved towards the other worker. The planks tilted to one side which caused both workers to fall approximately 40 feet to the ground.
