Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by falling equipment: a) M&M Well Service LLC, WCR 15 and Hwy I-25, Rig #5, Windsor, CO 80550: On and before July 2, 2017, the employer did not ensure that employees were protected from the hazard of being struck-by falling equipment. Employees were tripping out of a well with 521 joints of 2 7/8" pipe (16,095 feet) down hole. The crew was hoisting approximately 65,000 pounds, including 60,000 pounds of pipe and 5,000 pounds for the top drive. The 1" Bridon wire rope parted, and the top drive and wire rope fell to the rig floor. The employer did not ensure that the wire rope was in satisfactory working condition for hoisting this load. This condition exposed employees to the hazard of being struck-by falling equipment. Among other methods, feasible and acceptable abatement methods to correct the hazard include: 1) Ensure that a competent person inspects and replaces the wire rope in accordance with the manufacturer's directions. The Bridon North American Catalog includes a Trouble Shooting Guide, and provides instructions and directions for use on pages 48 - 61. Page 58 of this catalog provides that: "The entire length of rope should be inspected and particular attention paid to those sections that experience has proven to be the main areas of deterioration." 2) Ensure that a competent person inspects and replaces the wire rope in accordance with the API Recommended Practice 54 (1999 reaffirmed January, 2013) Section 9.6. Section 9.9.1 provides that "All hoisting lines should be visually inspected at least once each day. Hoisting lines should be thoroughly inspected once each month and a record made of the monthly inspection, designating any noted defects...."
