Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) The employer did not ensure that the control of hazardous energy procedure was utilized by employees who conduct set-up operations, cleaning functions and other servicing and maintenance work on the machines in the facility, including, but not limited to the Greenerd press machines. (b) On or about June 24, 2017, the employer did not ensure that an employee who was engaged in adjusting and cleaning a Greenerd press, machine # C1562, was utilizing the control of hazardous energy procedure. The employee suffered a partial fingertip amputation when a ram arm inadvertently moved in an upward direction, causing the injury.
