29 CFR 1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered: a) Wood Chipper; On or about June 26, 2017 the employer exposed employees to amputation hazards in that employees were allowed to walk and work around the wood chipper that had an unguarded projecting shaft end with keyways.
