Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about September 26, 2017, located on the Line 1 Assembly Machine, the employer did not include on their written control of hazardous energy procedures, (A) A specific statement of intended use of the procedure, (B) Specific procedural steps for shutting down and isolating all energy sources, specific procedural steps for placement removal and transfer of lockout devices and specific procedural steps for verification that the machine's energy sources were deenergized and at a safe state, thereby exposing the employees to the unexpected startup and/or release of stored energy. The lockout/tag procedures provided for the Assembly Machine did not include the hydraulics as a source of energy, the magnitude of the pneumatics, and specific procedures for the removal of lockout devices. The procedure also included pancake devices to be used when there were no gate valves on the machine.
