29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces. On or about June 7, 2017 at 2081 Eatonton Road, Madison, Georgia, the employer exposed an employee to entrapment and inhalation hazards by failing to evaluate all spaces to determine if they should be deemed a permit-required confined space prior to allowing employees to enter these areas for cleaning, maintenance, and inspections.
