29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface (see Figure D-1); a. On or about June 1, 2017, located at a residential home in Columbus, Ohio, the employer did not ensure that an extension ladder was three feet above the landing surface for employees to safely access to and from the ladder, thereby exposing the employees to a fall hazard of approximately 20 feet. On June 1, 2017, an employee fell approximately 20 feet while stepping from the eave of a roof and onto a ladder that was not three above the landing surface.
