29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at the Atlas Industrial Contractors, LLC worksite in Marysville, Ohio: a. On or about June 08, 2017, on the 2017 OSHA 300 Log, the employer did not provide all information required in column F such as a description of the injury or the source of the injury. 1. Case 2, March 16, 2017, Column F stated, "While lifting a megger tester onto the shelf of the conex, I heard and felt a pop in my shoulder", which was insufficient to determine what the actual injury was. 2. Case 4, May 15, 2017, Column F stated, "Walking across the 1st floor of new paint0line building, felt pop in right knee", which was insufficient to determine what the actual injury was. 3. Case 5, June 7, 2017, Column F stated, "Walking Two broken ribs, partially collapsed lung, and gash in arm requiring stitches", which was insufficient to determine what the actual source of the injury was.
