Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the Manitowoc crawler crane, model #4600, s/n 460061, which had not been tested and certificated by an accredited person or his authorized representative. Crane was used to hoist leg sections of an offshore jack-up drilling rig to be connected to leg already in place. Employees were exposed to the hazard of being struck by falling portions of the load or projectiles caused by falling load. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain written, dated and signed inspection reports and records of annual examination of cranes in accordance with Paragraph 5-2.1.3 of the USA Standard, B30.5-1968, crawler, locomotive and truck cranes. IN THE ALTERNATIVE: 29 CFR 1915.115(a)(1): Derricks and cranes which were part of, or regularly placed aboard, barges, other vessels, or on wingwalls of floating drydocks and were used to transfer materials or equipment from or to a vessel or drydock were not tested and certificated in accordance with the standards provided in 29 CFR Part 1919 by persons accredited for that purpose: a) GORILLA-V offshore jackup drilling rig, Sabine Pass, Texas 77655 Manitowoc crawler crane, model no. 4600, s/n 460061 was not tested and certificated by an accredited person or his authorized representative. Hazard: struck by falling portions of the hoisted load or projectiles caused by falling load; broken bones and/or death.
