29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 5/12/2017, the employees tasked with wiping off debris from the cleaning blades, did not conduct the following procedures to control hazardous energy as required by 29 CFR 1910.147(d)(1)-(6): 1) The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment, 2) All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s), 3) A lockout device was not attached to all power sources, 4) Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources, 5) Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished.
