Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to multiple broken bones and/or death: Worksite: Neither the "job-made" wooden platform, from which two(2) employees were working and which was lifted by the forks of the "Lull" material handling powered fork truck was equipped with standard guard rails nor were the two(2) employees wearing appropriate personal fall protection systems. Further, the platform was not positively secured to the forks or other structures of the truck. Among other methods, one feasible and acceptable method to correct the hazard is to: 1. Follow the requirements of the American National Standard Institute standard, ANSI B56.6- 1992, Safety Standard for Rough Terrain Forklift Trucks. 2. Follow the manufacturer's operating procedures.r's
