29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) Complete Energy Services Inc. at 5452 CO-1, Fort Collins, CO 80524: On and before May 1, 2017, the exposing employer did not ensure that truck drivers were instructed in the recognition and avoidance of unsafe conductions while operating vacuum trucks during pigging operations. The truck drivers were not trained on working with pigging crews to ensure pipeline pressures are de-energized or controlled prior to opening the receiver hatches. In addition, the truck drivers were not trained on safe positioning when opening the pig receiver hatches. This condition resulted in the injury and hospitalization of a truck driver when he was struck by three pigs when a pipeline receiver hatch was opened under pressure. This condition exposed vacuum truck drivers to the hazard of being struck by pigs under pipeline pressure.
