29 CFR 1910.147(c)(4): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Case Pick Structure - On or about 4/15/2017, the employer does not have specific procedures for locking out the Multilevel Vehicle Conveyor (MVC) during an employee confined space entry for removal of debris from an untenanted charging pad which exposed the employee to amputation and caught in hazard from the MVC which moves vertically and intersects with each level of the structure during the entry. - Up to 8 bots move on each level with a speed capacity of 26mph exposing employees to struck by hazards. - The Multilevel Vehicle Conveyor (MVC) moves vertically and intersects with each level of the structure, which presents an amputation and caught in hazard to employees maintaining the charging pads.
