29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 17, 2017, in the Reed Lane Debone Cooling Area at OK Foods, located in Fort Smith, Arkansas; the employer did not utilize the procedures for the control of hazardous energy, including but not limited to specific lockout/tagout (LOTO) for the removal of residual and/or re-accumulated Ammonia from the Evaporative Unit #12 during an emergency.
