29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices (RAGAGEP). Instance a) On or about April 17, 2017 and at times prior thereto, at Reed Lane engine room at OK Foods located in Fort Smith, Arkansas the employer did not follow RAGAGEP, such as IIAR Bulletin #111 (Ammonia Machine Room Ventilation) 2002, Section 4.1 and IIAR-2-1999 Section 6.2.2, when it did not include the following in its ventilation fans and intake testing: for inspection of the mechanical ventilation system in the Reed Lane engine room at OK Foods located in Fort Smith, Arkansas. Examples of such RAGAGEP include but are not limited to IIAR Bulletin #111 (Ammonia Machine Room Ventilation) and/or IIAR-2 (Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems). The employer failed to perform required testing for Ventilation Fans and Intake: a. Stop the non-emergency continuous ventilation fan(s) using the appropriate electrical disconnect(s). Confirm that the alarm for non-emergency continuous ventilation functions properly. b. Confirm that control circuitry, ventilation fans, dampers function properly. This hazardous condition exposed employees to toxic ammonia inhalation hazards. Instance b) This violation was most recently observed on or about April 17, 2017 at the Reed Lane ammonia refrigeration process when the employer failed to follow RAGAGEP such as IIAR Bulletin No. 109 Section 4.7.5 (Minimum Safety Criteria for a Safe Ammonia Refrigeration System) and IIAR Section No. 110 1993 (Guidelines for Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems), Section 6.7.2 when it did not inspect at least annually the insulated ammonia refrigeration piping such as the CD piping for the Recirculator Oil Pot and the insulation on Valve 196 on the engine room roof which as damaged/deteriorated. This hazardous condition exposed employees to toxic ammonia inhalation hazards.
