29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. (a) The employer is failing to protect employees from repeated exposure to hazards due to inadequate tracking. This was most recently documented on April 12, 2017, when the employer did not provide complete copies of the requested OSHA Injury and illness logs to the authorized representative with employee names. OSHA initially requested OSHA 300 logs for the last 5 years on April 12, 2017 at 1:29 pm. The employer provided OSHA 300 logs with the employee's names absent at 3:30 pm, on April 12, 2017. Complete OSHA 300 logs, with employee's names, were subpoenaed on May 25, 2017. A subpoena response was due June 26, 2017. The employer provided their partial subpoena response on June 27, 2017, absent employee names. A subpoena extension was granted until July 10, 2017. No response was received on July 10, 2017. The employer provided complete OSHA 300 logs on July 18, 2017. Abatement certification for this violation is not required
