Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of an inability of employees of Farrar OIl Field Service & Equipment Co., Inc. to immediately escape from the elevated monkeyboard, approximately 52 ft. above the ground, in the derrick of workover rig #4 (1981 Cardwell KB-200C with 96 ft. derrick) during intermittent or uncontrolled eruptions of either toxic hydrogen sulfide and/or flammable/combustible hydrocarbon gases from the R. & H. - Travis #3 Oil/Gas Well, which is known to have produced such gases, due to there being no emergency escape device. This occurred throughout he period between 4/24/90 to 5/29/90 at this well which is located in Section 6, TION, R9W, Jasper County, MS. Among other methods, one feasible and acceptable method to correct this hazard would be the installation of a property rigged and secured emergency descent device, or other satisfactory, auxiliary means of agress, such as specified in Section 4.7.1 of A.P.i. RP-54: Recommended Practices for Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations, First Edition, January, 1981, page 11. Essential elements of this system would include: 1) Staking out a securely anchored one-half (1/2) inch wire rope escape line attached to the derrick or mast so as to provide a ready and convenient means of escape from the derrickman's work platform(s). 2) The escape line route is to be kept clear of obstructions at all times. 3) A safety device is to be provided to permit fast emergency exit from the derrickman's work platform(s) to a safe place on the ground. 4) The tension on the escape line is to be periodically checked and adjusted to assure a safe landing and to allow the derrickman to stop twenty (20) to twenty-five (25) feet from the anchor point. 5) The ground anchor point of the escape line is to be located a minimum lateral distance from the derrick or mast equal to the height above ground to the point where the escape line is fastened to the derrick or mast.
