Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of not preventing the intrusion of formation fluids into the wellbore due to the failure of Farrar Oil Field Service & Equipment Co., Inc.'s crew of workover rig #4 to: 1) Displace wellbore contents prior to pulling tubing from well. 2) Add saltwater/powere fluid to the wellbore as tubing was being pulled from the R. & H. - Travis #3 Well located in Section 6, TION, R9W, Jasper County, MS. This hazard occurred on or about Tuesday, May 29, 1990. Among other methods, one feasible and acceptable method to correct this hazard would be: 1) Calculate the volume of the tubing and casing annulus, then pump that amount of saltwater, or other fluid of appropriate density, down both sides to displace any hydrocarbon contents before pulling pipe. 2) Add saltwater, or other fluid, to the wellbore during the tripping- out process to compensate for the weight of the tubing being pulled, and to maintain a hydrostatic overbalance, as specifiedin Section 3.13 and 4.4 of A.P.I. RP-59, Recommended Practices for Well Control Operations, First Edition, 8/20/87.
