Text For Citation: 02 Item/Group: 001C Hazard: CRUSHING
The alleged violations below have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) For the TMP Manlift on or about June 2, 1997, employee(s) were exposed to the hazard of being crushed between the step and the overhead steel platform when the safeties or limit switches were inoperable, b) For the Millwright manlift, on or about June 2, 1997, employee(s) were exposed to the hazard of falls and crushing injuries in the event the limit switches or safeties were inoperable, c) For the TMP manlift, employee(s) were exposed to the hazard of falls or other injuries in the event of failure of safeguards provided by items such as but not limited to guardrails, steps, step fastenings, belt and belt tension, handholds and fastenings, lubication, warning signs and lights, driving mechanism, brake, etc, d) For the Millwright manlift, employee(s) were exposed to the hazard of falls or other injuries in the event of failure of safeguards provided by items such as but not limited to guardrails, steps, step fastening, belt and belt tension, handholds and fastenings, lubication, warning signs and lights, driving mechanism, brake, etc. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...." for assuring operable safety stops/limit switches is to maintain a weekly inspection log or record that will provide a systematic work practice and help assure that the safety stops/limit switches are checked/inspected weekly as previously logged or recorded for the TMP and Millwright manlifts in March 3, 1994 and November 11, 1993, respectively; for assuring safety provided by other items such as but not limited to the mechanical, structural and other safety items listed above restart the monthly inspection log last used in December 1988. In addition, both the weekly and monthly inspection logs are required as stipulated in ANSI A90-1-1969 Rule 401(a). In The Alternative: 29 CFR 1910.68(e)(3): Written records of findings were not kept for each inspection of manlift(s): Maintaning the required certification inspection record is a work practice that helps insure that safeguards are in place: a) For the TMP Manlift, the weekly inspection log for limit switches was last recorded on March 3, 1994, b) For the Millwright Manlift, the weekly inspection log for limit switches was last recorded on November 11, 1993, c) For the TMP Manlift, the monthly inspection log for the many mechanical, electrical and other safety items was last recorded in December 1988, d) For the Millwright Manlift, the monthly inspection log for the many mechanical, electrical and other safety items was last recorded in December 1988.ber
