Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) For the TMP Manlift on of about June 2, 1997, employee(s) were exposed to the hazard of being crushed between the step and the overhead steel platform because the safeties or limit switches were inoperable, b) For the Millwright Manlift on or about June 2, 1997, employees were exposed to the hazard of falls and crushing injuries when carried over the top in the event the limit switches or safeties were inoperable. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...." for assuring operable safety stops/limit switches is to conduct weekly inspecting and testing(checking) of limit switches/safeties as previously conducted and ending on March 3, 1994 and November 11, 1993 for the TMP and Millwright Manlifts, respectively; or as stipulated in ANSI A90.1-1969, Rule 401(a) and Operator's Manual, calls for "hand pressure" test on the steps when reaching the limit switches. In The Alternative: 29 CFR 1910.68(e)(1): Manlift(s) limit switch(es) were not checked weekly: a) For the TMP Manlift, on or about June 2, 1997, employee(s) were exposed to the hazard of being crushed between the step and the overhead steel platform because the safeties or limit switches were inoperable and weekly records of inspecting and testing (checking) ended on March 3, 1994, b) For the Millwright Manlift, employee(s) were exposed to the hazard of falls and crushing injuries in the event limit switches or safeties were inoperable and weekly records of inspecting and testing (checking) of the limit switches ended November 11, 1993.ber
