Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about June 2, 1997 employees were exposed to the hazard of being crushed between the step of the "up-side" of the TMP-belt manlift and overhead steel platform when the two "up-limit" switches failed to cut off power to the belt drive. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...." to install an emergency escape grab-bar/rail and platform to permit the rider to swing free should the emergency stops (switches) become inoperative as stipulated in ANSI A90.1, 1969, Rule 106(C) and as depicted in National Safety Council Data Sheet No. 1-4-1-81, Manlifts, Fig.7. In The Alternative: 29 CFR 1910.68(b)(11)(iii): For belt manlift(s), an emergency grab bar of rail and platform was not provided at the head pulley, when over 6 feet above the top landing, to permit the rider to swing free should the emergency stops become inoperative: a) For TMP-belt Manlift, the head pulley and surrounding maintenance platform were 16 feet above the top landing and a grab bar/rail and platform were not provided for the rider's escape (swing free) in event of failure of the emergency stops (limit switches).
