Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) For the TMP Belt Manlift, 5th floor, on or about June 2, 1997, employee(s) were subject to crushing or falling hazards when employee(s) were unable to escape from the steps of the manlift when the upper limit switches fail or when the mechanical drive or power fail(s) and belt reverses on the down run. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS..." to install a fixed ladder accessible from both the up and down run of the manlift for the entire travel as stipulated by ANSI A90.1, 1969, RULE 107, as presently installed on this manlift for other floors except the 5th. In the Alternative: 29 CFR 1910.68(b)(12): The belt manlift was not provided a fixed metal ladder accessible from both the "up" and "down" run of the manlift for the entire travel of the manlift: a) For the TMP belt manlift, 5th floor, on or about June 2, 1997, employee(s) were subject to crushing or falling hazards when employee(s) were unable to escape from the steps of the manlift when the upper limit switches fail or when mechanical drive or power fail(s) and belt reverses on the down run by access to a fixed ladder as installed for other floors.ed
