Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards created by abrasive wheel failure on a Fox stand grinder, and employees being struck by parts of the wheel: On or about January 7, 2003, in the Cleaning Room located in Building 48, operator(s) using the Fox stand grinder, Model #G50S2, were exposed to the hazards of abrasive wheel failure as a result of: (a)Failure to provide a guard that would adequately contain large pieces of the abrasive wheel. (b)The employer allowing employees to improperly handle abrasive wheels during transport of them to the stand grinders. Employees were known to and observed rolling wheels from the storage rack to the grinders. (c)Not properly maintaining the lubrication of the sheaves on the #10 Fox stand grinder, to prevent use outside of tolerance limits and defects. (d)Use of 30" abrasive wheel operated at 2580 RPM when the manufacturers recommended safe operating speed for that wheel diameter was 1592 RPM. Among others, one feasible and acceptable method of abating the above conditions would be to: (a)Ensure that all stand grinding equipment is provided a self-closing guard in accordance with ANSI B7.1. (b)Provide for employee use a cart or other suitable means to transport the 30" abrasive grinding wheels. (c)Establish and implement a preventative maintenance program in accordance with the manufacturer's recommendations and American National Standards Institute (ANSI) B11.9. (d)Ensure the operators are fully trained on the employers grinding procedure to ensure they understand the reasons for and requirements of obtaining a speed test after each wheel change and at the beginning of the shift. In addition, the employer shall ensure that the speed testing is being accomplished in accordance with the wheel manufacturers recommended intervals and in accordance with their own program. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment
