29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: A) Metal Conversions, Ltd., located at 849 Crawford Avenue, Mansfield, OH 44905: On or about March 21, 2017, the employer did not develop and implement a written permit-required confined space program. Employee entered the bailer (manufactured by Harris Press & Shear, Inc., Model HRB8) in order to conduct activities such as, but not limited to, compacting the material and were exposed to the hazards associated with the bailers operation (amputation hazards). Written permit-required confined space program shall include elements such as, but not limited to: � Measures to prevent unauthorized entry � Hazard identification and evaluation � Means, procedures, and practices for safe permit entry operations including the specification of acceptable entry conditions, provisions for authorized entrants or their authorized representative to observe any monitoring or testing, isolation of the space, purging, inerting, flushing, or ventilating as necessary to eliminate or control atmospheric hazards, and verifying that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry � Necessary equipment � Evaluation of permit space conditions � Providing attendant(s) � Designation of active roles (authorized entrants, attendants, entry supervisors) in entry operations � Procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue.
